An Interview with Houston plastic surgeon Dr. German Newall's patient coordinator, Patty Escobar

As Dr. German Newall’s patient coordinator for 12 years, Patty Escobar works closely with patients by listening to their concerns, answering their questions, and educating them so they know what to expect at each step of their cosmetic plastic surgery experience. Her warm, caring attitude and upbeat personality provide reassurance and support to patients through a journey that is often transformative both physically and psychology.

In this interview, Patty shares insights about working with Dr. Newall and the goals they strive for in working together as a team.

Q: Patty, making the decision to have cosmetic plastic surgery is a big step. How do you go about putting people at ease during the initial consultation?

Patty: It is a big decision, and one that can feel a little intimidating initially. When people come to our office, they are seeking to improve a feature they feel self-conscious about.

However, Dr. Newall makes them feel comfortable immediately. He has compassion for his patients, and he’s very sincere. He gives them hope, and he gives them a solution to what they are looking for.

Q: Having a sense of trust is an important factor in choosing a plastic surgeon. How do you build that with patients?

Patty: It is very important to get that sense of comfort from your doctor and to have a connection. If you don’t need a procedure, he’s not going to tell you that you need one. He’s very flexible with his patients in that he gives them different options. He might tell them, “If you can’t do a tummy tuck right now, you could just have liposuction now and have a tummy tuck at a later date.”

It’s okay if we don’t book a patient for surgery. We would rather a person be happy and ready. People will say, “Thank you for being honest with me.” And they always come back. They appreciate Dr. Newall’s sincerity, and feel that he’s being honest with them.

Q: Dr. Newall has said before that he strives to create subtle change that transforms an individual into a more beautiful person rather than producing an overly dramatic change that creates a fake look. How does he strike that balance?

Patty: Cosmetic plastic surgery is his art, and he has an amazing aesthetic eye. He’s a perfectionist—every patient is going to get the best of who he is.

Related to this, it’s so important that there is vivid communication between Dr. Newall and a patient—that a person expresses her/his thoughts and aesthetic goals clearly so that Dr. Newall knows exactly what end results a person desires. You’ll never reach perfection, but he’ll try to achieve a person's goals as closely as possible.

Q: What are some of the most rewarding aspects of your job?

Patty: It’s such a privilege to come beside patients to give them a sense of caring and compassion. We hold their hands to the end, and they’re very grateful that they had that experience from the beginning to the end.

It’s extremely rewarding to see the positive difference it makes in people not only to the outward appearance but also to their confidence. Their personality and self-esteem is transformed between their first visit to the office until six weeks later.

They will often tell Dr. Newall, “You changed my life.” That is the ultimate reward for him.

U.S. Census Data Mirrors Increase in Latino Patient Base and Culturally-Influenced Consumer Preferences at Houston Plastic Surgery Practice

For Houston plastic surgeon Dr. German Newall, the exponential growth of the nation’s ">Hispanic population combined with Newall's increasing visibility on Spanish-language networks such as Univision have translated to a dramatic rise in his Latino patient base and a corresponding shift in consumer preferences.

Newall’s ability to converse fluently in both Spanish and English combined with his ease in front of the camera have made him a popular guest on many Houston-based Spanish-language television networks, newspapers, and radio stations. In addition, Newall caught the eye of television producers at Telemundo, who invited him to be the cosmetic plastic surgeon expert on the show Glamour Extremo, to be aired throughout the United States and Latin America. On the show, Newall discusses what can be done cosmetically to enhance the face and body from the perspective of a plastic surgeon, while a panel that includes a makeup artist, fashion stylist, and nutritionist offer their expertise to help transform the appearance and confidence of a selected individual.

These programs have heightened Newall’s visibility among the nation’s Hispanics, whose population in the United States has grown by 43 percent since 2000, according to 2010 U.S Census Bureau statistics. Census data shows Hispanics comprise 16 percent of the nation with a population of 50.5 million residents. Newall’s ability to converse in their language is a compelling factor that leads his Spanish-speaking patients to choose him as their plastic surgeon.

"The overall reason I think they feel more comfortable with me is the language barrier is removed," said Newall, who graduated from Georgetown Medical School in Washington D.C. as the first non-U.S. citizen to be accepted into the school’s program. “They can really express what they want and need, and I understand it perfectly.” This ability to communicate directly with the patient is more effective than having a third-party Spanish-speaking liaison translating between the physician and patient, he said.

Among the surgeon’s Hispanic patients, body contouring surgery such as liposuction, the Brazilian Butt Lift, and tummy tuck surgery are by far the most requested procedures, significantly outdistancing requests for facial rejuvenation procedures such as f acelift , browlift, and eyelid surgery.

One of the main reasons for this is Hispanic skin tends to be thicker and less easily affected by environmental factors like the sun, which can prematurely age the skin and facial appearance, said Newall. Cultural preferences also account for the popularity of body contouring procedures among this group. In general, women from South America prize having curves throughout the entire body, in contrast, to the more top-heavy, slim-hipped proportions that are favored in the United States, said Newall.

His Latin-American upbringing influences his own perspective of body contouring goals for female patients: to restore well-proportioned lines to the body while enhancing feminine contours. Whether it is breast augmentation, Brazilian Butt lift surgery, or a tummy tuck procedure, Newall incorporates subtle techniques that accentuate curves to give patients results that have an added “wow” factor.

"The concept of making a female more boyish by removing her natural curves is the wrong approach. The eye perceives beauty as a proportional whole,” said the surgeon."

In polls conducted by H Texas Magazine, Houston-area physicians have repeatedly named Newall one of the city’s top plastic surgeons to whom they would send family and friends. In addition, a 2010 Goldline Research study published in Forbes magazine listed the Houston physician as one of the leading providers of plastic surgery in the nation.

Liposuction: Better Body and Better Health? New Study Suggests Link

While liposuction is one of the most effective tools Houston plastic surgeon Dr. Newall uses to enhance the body’s contours, a new study shows it may also reduce the risk of heart disease, stroke, and diabetes.

Presented at the American Society of Plastic Surgery annual conference of 2011, the study revealed that liposuction patients experienced a reduction in triglyceride levels and white blood cell counts. Triglycerides are a type of fat found in the blood which the body uses for energy. While triglycerides are needed for good health, excessively high levels of this fat can raise the risk of heart disease.

The study measured the triglyceride and cholesterol levels of 322 patients who were undergoing liposuction and/or a tummy tuck. While triglyceride levels remained unchanged for those who had normal levels before surgery, patients who had elevated triglyceride levels (greater than or equal to 150 mg/dl) prior to surgery experienced a 43 percent reduction in triglyceride levels after surgery. This result is approximately twice the effect that is achieved with a drug that is commonly used to improve triglyceride levels.

In addition to a reduction in triglyceride levels, the study found that white blood cells decreased by an average of 11 percent after liposuction. High white blood cell counts have been linked to coronary heart disease, diabetes, and hypertension.

The study author, plastic surgeon Dr. Eric Swanson, noted that these findings suggest that subcutaneous fat that is found beneath the skin may have more metabolic significance and a stronger link to heart disease and diabetes than was previously thought. The visceral fat that surrounds the organs has long been considered to have the most direct link to these medical conditions.

While the study sheds light on the positive impact liposuction can have on the health, the body contouring procedure should not be considered a substitute for medications that are prescribed to reduce triglyceride levels.

Curves Ahead - The Comeback of the Curvy Figure

Curves are making a comeback thanks to such high profile actresses as Mad Men’s Christina Hendricks, Kim Kardashian, and singer Beyonce. But while this trend may be new for American women, Houston plastic surgeon and native Latin American Dr. German Newall says Latin Americans and Europeans have always prized feminine curves.

South American women, he said, are more concerned about having curves throughout the body, whereas the ideal in America tends to be more of a top heavy, slim-hipped figure. But with famously curvaceous celebrities such as Jennifer Lopez and Scarlett Johansson in the media spotlight, procedures to recontour and increase the buttocks through fat grafting are on the rise in America. Statistics published by the American Society for Aesthetic Plastic Surgery showed butt augmentations increased by 37.3 percent in 2009.

Dr. Newall says the trend reflects what he has always considered his body contouring goal, one that was influenced by his Latin American upbringing: to create a well-proportioned frame while enhancing feminine curves. Though he is considered an expert at performing large-volume liposuction to remove up to 22 pounds of fat, he makes an effort to preserve pleasing aesthetic proportions and contours, rather than focusing solely on fat removal. Selectively removing the fat allows the surgeon to reduce bulk where it is undesired while leaving behind right amount to achieve an ideal feminine or masculine shape.

“What attracts a males to females is what males do not have: curves, “ said the doctor, who left his native Peru at age 18 to attend medical school in the United States, and later received his plastic surgery training in Houston. “The concept of making a female more boyish looking by removing her natural curves is the wrong approach. Women of the 1950s and 1960s would have agreed, at a time when the physical ideal was represented by such voluptuous film stars Marilyn Monroe and Sophia Loren, the Italian actress who declared, “Everything you see, I owe to spaghetti.” This trend began to shift in the late 1960s when English model Twiggy, with her 5’6”, 91-pound frame ushered in an era of runways and fashion magazines featuring models with straight hips and fat-free frames. Though some received their thin figures due to genetics, for others it was the result of extreme calorie restriction.

The comeback of the hourglass figure represents a more attainable goal for many women, allowing them to embrace their natural curves while pursing a more balanced approach to exercise and diet.

Combination Body Contouring Procedure Achieves Dramatic Results Less Invasively

In his nearly 20 years of experience as a plastic surgeon, Houston physician Dr. German Newall, says that one of the procedures he has found to be the most effective at reshaping the body is a combination liposuction/abdominoplasty (tummy tuck) procedure. The results are often comparable to a more extensive body lift procedure, but the combination procedure has the advantage of being less costly and has a shorter and more comfortable recovery period, he said. While a patient who has an abdominoplasty/liposculpture procedure is typically able to resume a normal routine in 10-14 days, body lift surgery requires a longer recovery period of approximately four weeks.

“ For most individuals the removal of excess loose skin is required only in the abdominal area, which is accomplished through an abdominoplasty, while lipocontouring alone is sufficient to reshape the rest of the body,” said Dr. Newall. A tummy tuck restores a smooth, flat contour to an abdomen that has developed a loose, flabby appearance due to pregnancy, weight gain or loss, or genetic tendencies. Liposuction is employed to further reshape the body by removing excess fat from areas such as the waist, hips, thighs, and buttocks.

Dr. Newall said his typical patient has done everything in his/her power to lose weight, but has had little success because of genetic tendencies and inherent body shape. And patient response to the changes?

“They are overwhelmed with joy. “

Husbands who were previously opposed to the procedure, says Dr. Newall, often come to him afterward and relate that it was the best thing that ever happened to them. The changes to the body often produce a positive boost in self-confidence and self-image that impacts their relationship positively, as well.
